By entering your full name, email address and phone number you are providing personal information that will be used by New River Valley Periodontics & Implants for the sole purpose of returning your request to be contacted by us. We will only use this information to contact you in order to help you schedule an appointment with Dr. Habibzadeh, and to answer any questions you have indicated in the comments section. Our intention is to use your personal information only to return your request for contact about a dental appointment or a dental-related question.
Referrals sent by a dentist
The referral form is filled in by a dentist rather than by the patient, so the person it describes is not the person sitting at the screen. It asks for the referring dentist's name, the patient's name and phone number, the patient's address if the dentist has it, and the clinical detail behind the referral — periodontal therapy, implants, recession, ridge or sinus augmentation, crown lengthening, full-arch work, and any comments the periodontist should see before the appointment.
New River Valley Periodontics & Implants uses that information for one thing: to contact the patient, arrange the consultation and treat them. We do not use it for marketing, and we do not pass it to anyone outside the practice except where we are treating the patient or the law requires it. A dentist referring a patient is responsible for having the patient's agreement before sending their details.
The PDF copy of a referral is made by the dentist's own browser and saved to their computer. It is their record to keep, and it is not sent anywhere by us.
